The Yale Survivorship Program is the first dedicated, multidisciplinary resource in Connecticut for adult cancer survivors, and one of the oldest clinics of its kind in the country. A team of caregivers some of whom have survived cancer themselves provide patients with any type of cancer with support and guidance for going forward. The team reviews the patient's past cancer diagnosis and treatment history, including details of surgical procedures, types of chemotherapy and radiation therapy, and any treatment complications. Each patient receives an individualized treatment summary and survivorship care plan that is also shared with their primary care physician. The goal is for patients to leave the Survivorship Clinic with a better sense of direction and a feeling of empowerment. After visiting the clinic, many patients make important lifestyle changes and report that they are better able to cope with the emotions surrounding their experience.
The clinic's team meets with survivors in two visits to provide them with guidance and direction to help them maximize their health, quality of life and longevity. Patients see a medical professional, a nutritionist, an exercise therapist and a social worker as they undergo a focused evaluation with customized recommendations in the following areas:
