Dorothy Adler Geriatric Assessment Center
The Dorothy Adler Geriatric Assessment Center is an outpatient consultative service that provides a comprehensive assessment of older persons who have medical, psychological, cognitive or social problems impeding function or threatening independent living. Staff at the center work with the patient and family, the patient's physician, and other care providers in the evaluation to identify and coordinate community resources that will optimize the patient and family's living situation. Staff members include geriatric physicians, psychiatrists, case managers and patient care associates, all with special skills in the care of older persons.
Common problems addressed by the center include: loss of self-care skills, loss of mobility, difficulty walking, falls, memory loss, confusion, agitation, depression, complex medical problems and long-term care options.
