The Yale Pediatric Asthma Program is a multidisciplinary program that provides comprehensive care for children with asthma from infancy through adolescence. In addition to providing state-of-the-art evaluation, diagnosis, and management for patients with all degrees of asthma severity, the program addresses the psychosocial impact of the disease on patient and family dynamics by offering outreach services in the patient's home, when needed.
The Pediatric Asthma Care Team (PACT) is the group of providers from various disciplines who collaborate to ensure that all aspects of asthma care are addressed. The team includes pediatric pulmonologists, pediatric allergists, nurse practitioners, a certified asthma educator, a psychologist, a social worker, nurses and a bilingual outreach worker. PACT works closely with referring primary care providers to ensure that the lines of communication remain open and that any changes in the child's asthma care plan are known to all.
The Asthma Outreach Program (AOP) is the component of PACT that specializes in performing assessments of the home environment, as well as identifying barriers to optimal asthma control that may not become apparent during a clinic visit. Whether the barriers are psychosocial, environmental (at home or in school), or financial, the team works closely with the family to assist them in identifying the resources they need to overcome these barriers.
The pediatric asthma clinics at Yale are staffed by pediatric pulmonologists, pediatric allergists and nurse practitioners. Referrals to this program may include children with persistent asthma of any severity, as well as infants and children with chronic cough or wheezing that is unresponsive to therapy. A consultation includes a clinical assessment, pulmonary function testing, other testing as indicated, patient and family education regarding the use of inhaled medication, peak flow measurement, trigger avoidance and environmental control measures. When appropriate, a referral to the AOP is made.
