Urology Surgery

Yale Urology comprises physicians whose express purpose is excellence in the treatment of urologic disease. Each physician has developed clinical expertise in subspecialty disciplines within urology to provide comprehensive treatment of even the most complicated urologic problems. The strong academic and research backgrounds of the group assure a cutting-edge approach to treatment.

Urologic oncology encompasses the diagnosis and treatment of urologic malignancies of the bladder, prostate, testis, kidney, and penis. For both male and female patients with bladder cancer, the section offers, where indicated, intestinal neo-bladders to preserve a more normal voiding pattern. Specific expertise in continent urinary diversion by means of a Hautmann or Indiana pouch, depending on the particular clinical situation, also is available. A nerve-sparing radical prostatectomy is commonly utilized for localized prostate cancer however, for the patient desiring alternative treatment, a program for template radioactive seed implantation has been developed. Many kidney tumors are removed using laparoscopic techniques and robotic nerve-sparing radical prostatectomy is an alternative to open prostatectomies. Retroperitoneal lymph node dissections for the management of testicular cancer are performed with nerve-sparing to preserve ejaculation, or for the removal of residual tumor following chemotherapy.

Urology offers treatment and prevention of kidney stones and extracorporeal shock wave lithotripsy (ESWL) treatment. Patients who undergo this therapy or surgical treatment are monitored by physicians from the sections of urology and endocrinology to ensure that the cause and prevention of stones is understood. Prevention through nutrition, counseling, and medical management is individualized for each patient.

The section utilizes laparoscopic techniques such as laparoscopic nephrectomy and adrenalectomy. The length of stay with these procedures has proven to be significantly lower than for surgery using traditional methods.

The Neuro and Female Urology Program provides care for voiding dysfunction, incontinence, female urologic problems, and urinary tract difficulties related to neurologic diseases. When required, complex evaluations with fluoroscopic cystometry, urethral profilometry, uroflowmetry, and electromyography are performed. Varied treatment options include pharmacotherapy, behavioral modification, and surgery. Recently, minimally invasive surgical procedures, such as collagen implants, have increased the therapeutic alternatives.

Infertility is evaluated and treated utilizing microsurgical and minimally invasive techniques for the management of diminished fertility caused by varicoceles, obstruction of the vas deferens or epididymis, and dysfunction of sperm maturation.

Pediatric urology specialists care for children with urologic diseases, such as congenital anomalies, including hypospadias, undescended testes, hydronephrosis and exstrophy of the bladder, and they have expertise in the management of urologic neoplasms, including Wilms' tumor. Laparoscopic modalities are used for the management of intra-abdominal undescended testes and for a variety of renal abnormalities. Expertise is available for the management of neurogenic bladders associated with spina bifida.
